When a Tooth Needs to Come Out

Most of the time, saving a tooth is the better outcome, and your general dentist will try that first. Extraction is the right call when the tooth is too damaged or compromised to be restored, or when leaving it in place would put the surrounding teeth at risk.

Common reasons an extraction is recommended: 

• Decay that has gone too deep to repair with a filling or crown
• A tooth broken or fractured below the gum line, often after an injury
• An infection that has not resolved with other treatment
• Advanced gum disease that has loosened the tooth
• Crowding, or a tooth that has not erupted properly
• Clearing space before implant, denture, or full-arch treatment

The Kinds of Extractions We Perform

Not every extraction is the same procedure. Which one applies depends on the tooth, its position, and its condition.

Single Tooth Extraction

One tooth was removed because decay or damage had made it irreparable. For a fully erupted, intact tooth, this is usually straightforward.

Multiple Extractions

Removing several teeth in one visit is common, often ahead of a denture or full-arch plan. Planning them together can mean one recovery period instead of several.

Surgical Extraction

When a tooth is broken at the gum line, badly curved at the root, or hasn’t fully erupted, removal requires a surgical approach. This is the work an oral and maxillofacial surgeon is specifically trained for.

Extraction With Bone Grafting

What to Expect, Start to Finish

1. Schedule Your Appointment

If you have a painful tooth or your dentist has referred you for an extraction, call us. We will set up a consultation and answer your questions before you come in. Referring providers can use our doctor referral form.

2. Your Consultation

We review your concerns, examine the tooth, and take X-rays or other imaging as needed. Then we walk you through your options, including whether the tooth can be saved, so you can make an informed decision.

3. Planning Your Comfort

We offer a range of sedation options, including nitrous oxide, oral sedation, and IV sedation. Dr. Richard Montandon, our anesthesiologist, supports anesthesia planning, and what is appropriate depends on your medical history and the recommended procedure. If you have sedation, arrange for someone to drive you home.

4. The Procedure

On the day, we explain what will happen before we begin. Straightforward extractions are typically quick. Surgical extractions take longer and may involve a small incision to reach the tooth.

5. Recovery and Follow-Up

You will rest in our recovery area and leave with written aftercare instructions covering how to protect the site, manage discomfort, and watch for anything that needs attention. We generally schedule a follow-up to confirm you are healing as expected.

Healing After an Extraction

Most people are surprised by how manageable recovery is, particularly for a straightforward extraction. A blood clot forms in the socket to protect it as it heals, which is why the first day or two matter most and why you should follow your instructions carefully.

Your written aftercare instructions will cover what to eat, how to keep the area clean, what to avoid, and how to manage discomfort. Call us if something looks or feels wrong, including bleeding that will not settle, pain that increases after a few days, or signs of infection. We would always rather hear from you than have you wait it out.

Tooth Extraction Questions

Does a tooth extraction hurt?

We numb the area before the procedure begins, and we offer nitrous oxide, oral sedation, and IV sedation depending on your health history and what is being done. Most discomfort comes during healing rather than during the extraction itself, and your aftercare instructions cover how to manage it.

How long does healing take?

It depends on the tooth, whether the extraction was surgical, and your own healing. Your surgeon will give you a realistic timeline for your case at your consultation rather than a general figure.

Can I get an extraction the same day if I am in pain?

Call the office closest to you and say that it is urgent. We will tell you what we can do and how quickly. Whether removal happens the same day depends on the tooth and on what the examination shows.

Will I need a bone graft at the same time?

Sometimes. If an implant is part of your plan, placing a graft in the socket during the extraction can preserve the bone that supports it. Your surgeon will tell you whether that applies to you.

Do I need a referral from my dentist?
