Dr. Daniel Cullum
Oral & Maxillofacial Surgeon
Dr. Cullum completed his DDS with distinction at the University of Alberta, Canada and a general practice residency and specialty training in Oral and Maxillofacial Surgery at Westchester Medical Center, New York.
He is a diplomat of the American Board of Oral and Maxillofacial Surgeons. Dr. Cullum is on faculty as a Visiting Lecturer at both Loma Linda University and UCLA, Departments of Oral and Maxillofacial Surgery and is Past-Chair, Department of Surgery, at Kootenai Medical Center.
He speaks internationally and has written numerous articles, textbook chapters and a new textbook titled “Minimally Invasive Dental Implant Surgery.” Dr. Cullum has served in national, state and local leadership roles.
At Implants Northwest, he practices oral and maxillofacial surgery with an emphasis on patient-centered care utilizing immediate and minimally invasive techniques in esthetic implant reconstruction. Dr. Cullum has developed a number of innovative techniques and continues to teach and study internationally. Dr. Cullum is also the president of Implants Northwest LIVE Learning Center, a continuing education institute in Coeur d’Alene.
Dr. Cullum and his wife live in the Coeur d’Alene area. He loves the outdoors and especially enjoys skiing and spending time with his family.

Dr. Matthew Clark, DMD
Board Certified Oral and Maxillofacial Surgeon
Dr. Clark was born and raised in Vernal, Utah and grew up loving
all the recreation the diverse high mountain desert landscape had
to offer. He completed an associate’s degree at Utah State
University then transferred to the University of Tennessee in
Knoxville, TN where he completed his undergraduate degree in
Biochemistry and Molecular Biology. After graduation he worked for
a full scope Oral & Maxillofacial Surgery practice as an assistant while applying to dental school. He was accepted to the University of Kentucky College of Dentistry and attended on a scholarship with the United States Air Force.
Upon graduation he was commissioned as an officer with the rank of Captain and attended
Commissioned Officer Training. He was accepted into the San Antonio Military Oral &
Maxillofacial Surgery residency program where he completed rotations in internal medicine,
adult and pediatric anesthesia, general surgery, trauma surgery, ENT, critical care,
neurosurgery, burn plastic surgery and oral & maxillofacial surgery. Dr. Clark trained at several facilities including Brooke Army Medical Center, Wilford Hall Ambulatory Surgical Center, and the Texas Medical Center in Houston, TX.
Dr. Clark’s next military assignment sent he and his family to beautiful coastal Virginia where he promoted to the rank of Major and functioned as the department Chief of Oral & Maxillofacial Surgery at Langley Air Force Base Hospital. He held an appointment as a faculty member of the Langley Air Force Base Advanced Education in General Dentistry Residency program as well as the Portsmouth Naval Medical Centers Oral & Maxillofacial Surgery Residency.
He practices the full scope of oral and maxillofacial surgery while training the next generation of military dentists and surgeons. During his time as a full scope surgeon Dr. Clark completed numerous complex cases required to become a fellow of the American College of Surgeons. For the past nine years he had the privilege of providing care for modern warfighters such as military fighter pilots, cybersecurity experts, intelligence analysts, special forces operators, crew chiefs, drone pilots, medics, translators, and many others who deploy downrange in the fight to maintain our country’s freedom.
Currently Dr. Clark is a Diplomate of the American Board of Oral and Maxillofacial Surgeons, the American Association of Oral & Maxillofacial Surgeons, the Virginia Society of Oral & Maxillofacial Surgeons, and the American Dental Association. Dr. Clark has worked outside the military in private practice at Hampton Roads Oral & Maxillofacial Surgery, and Clear Choice Dental Implant Center focusing on same day smile delivery with dental implants. During his three years working at Clear Choice he completed over 500 full arch cases and has performed thousands of dental implant surgeries
Outside of his professional training and surgical
practice Dr. Clark is married to his wife Sheena of 16 years and they have three sons: Wyatt (11), Grayson (7), and Kasey (4). Dr. Clark loves raising his boys, all things with motors, and the outdoors. He can be found working on his classic car, riding his dirt bike, snowmobiling, skiing, backpacking, and coaching his sons’ baseball teams. He also took on a new challenge in 2025 and has been training in Brazilian jiu-jitsu with his oldest son Wyatt.
Dr. Clark is dedicated to caring for patients and ensuring they feel heard and understood. He and his family are so excited to move to Coeur D’Alene, Idaho and serve within the beautiful community.
Dr. Richard Montandon
Anesthesiologist
In order to provide the best in patient care, we often use a dental anesthesiologist for advanced anesthesia and pain management. Dr. Montandon works with Dr. Cullum to provide patients with a comfortable and safe surgical experience.
Dr. Richard Montandon was accepted into the Idaho Dental Education Program and received his Doctor of Dental Surgery (DDS) through Creighton University in Omaha, Nebraska. Immediately following dental school, Dr. Montandon was introduced to dental anesthesiology through a one-year general practice residency at Indiana University. He then completed an additional four years of intensive anesthesia residency training, which included anesthesia care for a broad scope of medical and dental procedures.
Upon completion of his anesthesia residency at the University of Pittsburgh Medical Center (UPMC), Dr. Montandon became an assistant professor at the University of Pittsburgh. During this time, he provided anesthesia care and trained dental anesthesia residents, student nurse anesthetists, and dental students. Dr. Montandon’s professional focus is primarily clinical dental anesthesiology; however, he has also been involved in lecturing, publishing articles, and other scholarly activities. Dr. Montandon is board certified through the American Dental Board of Anesthesiology and the National Dental Board of Anesthesiology and is a Fellow of the American Dental Society of Anesthesiology.
Dr. Montandon is dedicated to increasing the availability of dental and oral and maxillofacial surgery services to patients that are unable to have their treatment completed without the use of sedation or general anesthesia. Dr. Montandon is committed to helping meet each patient’s needs in a safe and comfortable manner.
After being out of the Inland Northwest region of the country for 11 years, Dr. Montandon and his wife are happy to be back home. In his spare time, Dr. Montandon enjoys his family and friends and spending time in the great outdoors.
